Before a driver can get their commercial driver's license they have to pass both an MTO medical exam and a Ministry of Transportation (MTO) physical. The MTO medical form for commercial drivers is completed by your doctor and sent directly to the ministry. Once approved, it will be issued with your new license. What do I need?: For the medical exam: A valid Ontario Driver's Licence or ID card and A current signed Medical Report Form.
What happens if I fail? If you fail any part of the medical examination, or if there are any other indications that might suggest that you may not meet medical fitness requirements to operate a vehicle as a commercial driver, then MTO may request additional information from your physician or require follow-up testing.
